Breast appearance changes over time due to multiple factors. Pregnancy and breastfeeding can significantly alter breast volume and elasticity, often leaving breasts deflated or drooping after weaning.

Weight fluctuations cause the breast tissue to expand and contract, stretching the skin and reducing firmness. As we age, natural collagen loss and gravity’s effects lead to sagging and loss of upper pole fullness.

Genetics also play a role in determining breast shape, size, and how they respond to these life changes. Understanding these causes helps in selecting the most appropriate surgical solution for your specific situation.

Breast Augmentation

Breast augmentation uses implants to increase breast size and improve overall breast shape. This procedure is ideal for women who desire fuller breasts, want to replenish volume lost after pregnancy or weight loss, or wish to achieve better body proportions.

During the procedure, implants are carefully placed either behind the breast tissue or beneath the chest muscle, depending on your anatomy and desired outcome. The surgery typically takes 1-2 hours and is performed under general anesthesia.

Breast augmentation may be right for you if you:

  • Desire larger, fuller breasts
  • Have lost breast volume after pregnancy, breastfeeding, or weight loss
  • Want to improve breast symmetry or proportion
  • Feel your breasts are too small for your body frame
  • Are in good overall health and have realistic expectations
  • Are finished with childbearing (though pregnancy after augmentation is possible)

During your consultation, we’ll assess your anatomy, discuss your goals, and determine if augmentation is the most appropriate procedure for achieving your desired outcome.

Submuscular (Under the Muscle): The implant is placed beneath the pectoralis muscle. This placement typically provides more natural-looking results, especially for patients with minimal breast tissue, and may reduce the risk of certain complications. Recovery may take slightly longer due to muscle involvement.

Subglandular (Over the Muscle): The implant is placed between the breast tissue and chest muscle. This placement may be suitable for patients with adequate breast tissue coverage and can result in a shorter recovery time. It may provide more pronounced upper pole fullness.

Dual Plane: A hybrid approach where the upper portion of the implant sits under the muscle while the lower portion sits under the breast tissue. This technique combines advantages of both placements and is often used for optimal positioning and natural appearance.

The best placement for you depends on your anatomy, breast tissue amount, lifestyle, and aesthetic goals. We’ll discuss all options during your consultation.

Saline Implants: Filled with sterile salt water solution. These can be adjusted for size during surgery and typically cost less than silicone. If rupture occurs, the saline is safely absorbed by the body. May feel slightly less natural than silicone.

Silicone Gel Implants: Filled with cohesive silicone gel that closely mimics natural breast tissue feel. Available in various consistencies from soft to highly cohesive (“gummy bear” implants). Generally provide the most natural look and feel. Require regular monitoring.

Shape Options:

  • Round implants: Provide fullness throughout the breast, especially in the upper pole
  • Teardrop (anatomical) implants: Shaped to mimic natural breast contour with more volume at the bottom

Surface Texture: Smooth or textured surfaces are available, each with specific advantages. We’ll recommend the best option based on your goals and placement choice.

Inframammary Incision (Breast Fold): Made in the natural crease beneath the breast. This is the most common approach, providing excellent surgical access and control while hiding the scar in the natural fold. Ideal for most implant types and sizes.

Periareolar Incision (Around Nipple): Made along the border where darker areola skin meets lighter breast skin. This placement can camouflage scarring well but may carry slightly higher risks of sensation changes or breastfeeding impact. Suitable for smaller implants.

Transaxillary Incision (Armpit): Made in the armpit crease, leaving no scar on the breast itself. Requires specialized technique and may limit implant options. Best for patients who want to avoid any breast scarring.

Transumbilical Incision (Belly Button): Rarely used approach with limited applicability. Only suitable for saline implants and provides less surgical control.

Breast Augmentation Before & After

Breast Lift (Mastopexy)

A breast lift repositions sagging breasts to a higher, more elevated position on the chest wall. This procedure removes excess skin, reshapes breast tissue, and repositions the nipple and areola to create a more elevated, firmer breast contour.

Breast lift surgery is particularly effective for addressing drooping caused by pregnancy, breastfeeding, aging, or weight loss. The procedure typically takes 2-3 hours and results in perkier, more shapely breasts without the use of implants.

Breast lift surgery may be right for you if you:

  • Have breasts that sag or droop (ptosis)
  • Notice your nipples pointing downward or falling below the breast crease
  • Have stretched skin and reduced breast firmness
  • Experience breast flatness or elongation at the top
  • Have asymmetrical breasts where one hangs lower than the other
  • Are satisfied with your breast size but want improved position and shape

Ideal candidates are in good health, maintain stable weight, and have realistic expectations. If you’re planning future pregnancies, you may want to postpone this procedure as pregnancy and breastfeeding can affect results.

Grade 1 (Mild Ptosis): The nipple is at the level of the inframammary fold (breast crease) with minimal drooping. May be addressed with a less extensive lift technique or even augmentation alone in some cases.

Grade 2 (Moderate Ptosis): The nipple falls 1-3 cm below the breast fold, with noticeable sagging. This typically requires a standard breast lift procedure with moderate tissue repositioning.

Grade 3 (Advanced Ptosis): The nipple falls more than 3 cm below the breast fold and points downward. This requires a more extensive lift procedure with significant tissue reshaping and skin removal.

Pseudoptosis: The lower breast tissue sags below the fold, but the nipple remains above it. This condition may sometimes be corrected with augmentation rather than a full lift.

During your consultation, we’ll assess your degree of ptosis and recommend the most appropriate surgical technique to achieve optimal results.

Crescent Lift: Minimal incision along the top half of the areola. Suitable for very minor lifting needs, often combined with augmentation. Results in minimal scarring but limited lift capability.

Periareolar (Donut) Lift: Circular incision around the entire areola. Appropriate for mild to moderate sagging. Provides moderate lift with scarring concealed at the areola border. Can also reduce areola size if needed.

Vertical (Lollipop) Lift: Incision around the areola plus a vertical line down to the breast crease. Suitable for moderate sagging. Allows for more significant reshaping and lifting while limiting horizontal scarring in the breast fold.

Inverted-T (Anchor) Lift: Incision around areola, vertical line down, plus horizontal incision in the breast crease. Best for severe sagging requiring maximum tissue removal and repositioning. Provides most dramatic results but with more extensive scarring.

The technique we recommend depends on your degree of sagging, breast size, skin elasticity, and desired outcome. We’ll discuss which approach is best suited to your specific situation.

Breast Lift Alone: Best for patients who are happy with their breast size but want to improve position, shape, and firmness. The lift removes excess skin and reshapes existing tissue without adding volume. Your breast size may actually appear slightly smaller after surgery due to tightening, though the overall appearance is fuller and more elevated.

Breast Lift with Augmentation: Ideal for patients who want both improved position AND increased size. This combination addresses sagging while adding volume for fuller breasts. It’s particularly beneficial if you’ve lost significant volume in the upper breast or want to achieve larger, lifted breasts in one surgery.

Dr. Qadir Khan

Mr Khan then moved to Perth, Australia in 2007. He was selected for the highly competitive specialist training programme in Plastic, Reconstructive & Aesthetic Surgery in 2014. After the successful completion of his fellowship with the Royal Australasian College of Surgeons in early 2019, he travelled to Brisbane and Newcastle Upon Tyne, England for post-training fellowships in Head & Neck Surgery, Microsurgery, Facial palsy and Sarcoma.
